Terms and Conditions: By submitting this information, you agree to the Terms and Conditions set forth in this Agreement. You acknowledge you are of legal age, have provided true information, are fully authorized on behalf of the entity requesting Service. By submitting you also authorize us to place all charges for Service on the billing method you selected with no additional notice or consent required. We make no express warranty regarding the Service and disclaim any implied warranty including any warranty of merchantability or fitness for a particular purpose. If I select NickelTalk for Switched Residential service, I appoint Nickeltalk and/or STS Telecom (Carrier) to act as my agent in all matters to and providing carrier selection for long distance service. I hereby authorize any Local Exchange Company (LEC) to make pertinent information available to Carrier for this purpose and follow Carrier's instructions with reference to any long distance service order or change which the LEC provides to Customer, and hereby releases Carrier and such LEC from any and all liability for doing so. Customer's use of any common carrier transmission service provided by Carrier constitutes acceptance of the terms and conditions of distance carrier. I have read and agree to the terms and conditions set forth herein and represent that I am authorized to execute this agreement on behalf of Customer.
